Dear students,
I hope your finals are off to a great start. Last week, Provost Prakash sent out a
message approving vaccination requirements for several high-contact, high-duration
lab courses. Please refer to this list of labs with vaccine requirements to see if your course is listed.
If you are enrolled in or plan to take any of the courses listed, please be aware
that you must be fully vaccinated (i.e., two weeks have passed since your final vaccine
dose) or have a university-approved medical or religious waiver by Jan. 21, 2022.
Here are the steps you need to take to ensure you remain eligible for your spring
labs.
1. Provide proof of vaccination
- Contact an authorized verifier with proof of vaccination and picture ID
- Proof can be: CDC vaccination card, a copy of your vaccination record from your medical
provider, or a foreign vaccination record that meets the World Health Organization's
standards
OR
2. Provide proof of an approved nonmedical or medical exemption
To be fully vaccinated by Jan. 21, 2022, you must:
- Receive the Johnson & Johnson vaccine no later than Jan. 7, 2022.
- Receive the first dose of the Pfizer vaccine by Dec. 17, 2021, and the second dose
by Jan. 7, 2022.
- Receive the first dose of the Moderna vaccine by Dec. 10, 2021, and the second dose
by Jan. 7, 2022.
UAF will host free COVID-19 and flu vaccination clinics in the Patty Center Dec. 7-8 and 14-15 from noon-8 p.m. The Student Health and Counseling
Center also offers both vaccines.
