Report Concerns
All concerns related to the protection of the rights and welfare of human research participants in University of Alaska Fairbanks (UAF) projects or the confidentiality of research data should be reported to the Institutional Review Board (IRB). To facilitate reporting, the IRB has adopted a report template (WORD); however, you may also express your concerns in-person, by telephone or email to any member of the Office of Research Integrity (ORI) or the IRB.
The ORI receives all reports on behalf of the IRB and handles the initial review and fact finding process. Although anonymous reports are accepted, the ORI encourages individuals to identify themselves and to make themselves available to answer follow-up questions should they arise during the review. To the maximum extent possible the ORI will protect the identity of the individual(s) submitting reports, but depending on the nature of the report and subsequent procedings this may not always be possible.
UAF protects employees from reprisal due to good faith reporting as required under the Alaska Whistleblower Act (AS 39.90.100-150). Student whistleblowers are protected as "laboratory personnel" under the Animal Welfare Act and under UA Board of Regents Policy (P09-01.09 (WORD)).
For more general concerns related to the offices under the Office of the Vice Chancellor for Research, there is an online submission option available here. You may submit comments regarding the Office of the Vice Chancellor for Research, Office of Research Integrity, Office of Intellectual Property and Commercialization, or Animal Resources Center.
