Use the form to report incidents of discrimination based on the protected categories such as: age, disability, equal pay/compensation, genetic information, harassment, national origin, pregnancy, race/color, religion, retaliation, and sex including sexual misconduct. Any individual may make an anonymous report regarding an act of sexual misconduct, including sexual harassment, sexual violence, stalking and intimate partner violence. For issues on discrimination based on sex, you may report the incident without disclosing your name, identifying the respondent or requesting any action. Depending on the extent of information available about the incident or the individuals involved, however, UAF’s ability to respond to an anonymous report may be limited. The Title IX coordinator will receive all reports submitted via this form and will determine appropriate steps including individual and/or community remedies.

- Discrimination reports can be filed simultaneously with external agencies.
- Individuals who believe they have suffered discrimination may use UAF’s internal resolution process and/or file a complaint with organizations external to UAF. The Diversity and Equal Opportunity office does not collect IP addresses or attempt to notify reporters.
