Registration Process

Here is what you need to do to register with our office

The first step is to complete our Registration Form. Our registration form covers a broad range of information. Some things may apply to you, and some items may not. You may skip any questions you feel do not apply to you. There are no right or wrong answers. This form helps us understand your needs better, and how we might be able to accommodate you.

The next step is for you to provide us with some sort of medical documentation. There are three different ways for you to do this. Please note that you only have to complete one of the options below.

  • Send us any previous medical documentation from a medical provider stating your disability, diagnosis, and/or symptoms you are experiencing. 
  • Have your provider complete our Verification Form, stating your disability or condition.
  • Alternatively, you can complete our Release of Information Form, and send the completed version back to us. By completing this form you are giving UAF Disability Services permission to request documentation from or communicate with whoever you designate on your behalf. This form should only be filled out if you wish to release confidential information. 

Once we have received both the registration form and documentation of your disability and you are registered for classes, our office will reach out to you to schedule an intake appointment. During this meeting we will get to know one another, and discuss what accommodations could help you be successful in your academic career. Intakes usually take around 45 minutes and can be done in person, over the phone, or via Zoom at your request. 

After the intake appointment, we will draft a letter stating your accommodations, and email it to your instructors at the start of the semester (with your permission of course). You must contact Disability Services to renew your letter every semester. Restoring your accommodations can be done as a simple phone call, email, or in-person visit. If you wish to change your accommodations, please reach out to us, and we will schedule another meeting.
