Biometric screenings available on UAF campus

Biometric screening will be offered on June 16 and 17, from 7:30 a.m. to noon both days, in the Butrovich Building, Room 109 (Board of Regents Conference Room). This will be the last on-site screening event before the June 30 deadline to qualify for the University of Alaska's fiscal year 2015 wellness rebate.

To register for the screening:
1) Go to my.blueprintforwellness.com.
2) Enter the registration key: UOAonsite.
3) Enter your username, using the first three letters of your first name, your full last name, two digits for your birth month and two digits for your birth day. (For example, George Clooney would enter geoclooney0506 as his username.)
4) Enter your date of birth.
5) Select the Fairbanks event and schedule your screening.

The screening requires fasting, so don't eat for 9-12 hours before your appointment. Drink plenty of water and take your regular medication. Don't delay, as the available appointments will fill up quickly and "walk in" spots will be limited.

The biometric screening is just one of the two activities you need to qualify for the $600 wellness rebate. Be sure to also complete the Personal Health Assessment at Healthyroads.com. Employees should use the Single Sign-On feature, using your UA username and password, with the link from the benefits website. Spouses (including employees who have opted out and are covered under their UA-employee-spouse) should go directly to Healthyroads.com to register and complete the PHA.
