Campus biometrics screenings Dec. 14-15

The University of Alaska is again offering on-site biometric screenings for all employees,
spouses and FIPs currently enrolled in the UA Choice health plan. Those who have not
already participated in a biometric screening for FY18 (5/1/17—4/30/18) at a Quest
Patient Service Center or through your health care provider will be able to participate.
Be sure to check your Healthyroads account beforehand. If you met the biometric outcomes with your FY17 biometric screening, you are not required to complete a screening
this year, although you are welcome to do another one this year.
Appointments must be scheduled online. All up-to-date information regarding the screenings is also posted on the www.UA benefit/wellness/ site.

Unique ID = First three letters of first name + Full last name + MMDD of your date

If you can't attend either of these events, there are options for getting the biometrics
completed. All details for these are provided on the www.UA benefit/wellness/ page; just scroll until you see "Biometrics Screening Information."
- Quest Patient Service Center (PSC)
- Schedule an appointment with your healthcare provider for an annual preventive exam, including the biometrics. You will need to take along the healthcare provider form to be completed and then return it to Healthyroads.
