The university now offers incentives for participation in the Healthyroads Program, provided to employees, spouses and partners enrolled in the UA Choice health plan.

Enrolled employees, spouses and financially interdependent partners are eligible to earn a rebate of up to $600 each on their health plan payroll contributions. Simply complete a personal health assessment and get a biometric screening by June 30, 2014. Participants will be entered into a raffle drawing beginning, March 31. The earlier you get your health assessment and biometric screening in, the more chances you'll have to win!

Employees should go to www.alaska.edu/benefits to access Healthyroads using your employee ID and password. Spouses and financially independent partners should go to www.Healthyroads.com and follow the prompts to create a username and password.

Biometric screenings
By either attending an onsite screening, visiting your health care provider or getting a screening at a local contracted facility, you can learn vital health statistics, including:

  • Blood pressure

  • Lipid panel (total cholesterol, HDL, LDL and triglycerides)

  • Body mass index and/or waist circumference

  • Glucose/blood sugar level

You can have your screening done at a time convenient for you by going to a Quest Patient Service Center near you. Go to my.blueprintforwellness.com and use the registration key of UOAremote. Follow the prompts to schedule your screening. Your results will be sent directly to Healthyroads, where your personal scorecard will be updated with the information.

On-site screenings will be scheduled in rural areas starting in early May. More events will be scheduled at other campuses in May and June.

Once your biometric screening is submitted, it will automatically populate your personal health assessment so you can generate a plan customized to you and your health needs. Employees who already have completed a biometric screening after July 1, 2013, can download the health care provider form (PDF), give it to your provider and have them submit your results. You can also access the health care provider form by visiting www.alaska.edu/benefits.
