UA creates new reporting site for accidents, incidents

Incident reporting has now been standardized throughout the University of Alaska system.
Effective immediately, incidents at UAF should be reported through the new online
Origami Risk program at http://www.uaf.edu/safety/occupational-safety/accident-reporting/.
Reports should be filed on injury or non-injury incidents involving UA employees (including
student employees) and non-employees (including students). Damage to UA and non-UA
vehicles and property should also be reported. Unsafe conditions can also be reported
via the site.
All reported incidents must be entered into Origami Risk. Paper or electronic accident/incident
reports should no longer be submitted to UAF Environmental, Health, Safety, and Risk
Management.

In an emergency, do not fill out the form; call 911.
With the new online process, employees no longer must complete the two additional
workers' compensation forms when an incident with injury results in medical attention.
