The state of Alaska worker’s compensation division has published new forms for reporting a work injury or illness. Effective July 22, 2013, all Alaska employers and employees must use the new forms to report a claim of work injury or illness. The most notable change requires that now both the employee and their employer (supervisor) complete separate forms for each claim of injury or illness.
